Acute Disease Management

($200/visit, includes any in-office testing and treatment if available in office, any send-out labs will be offered at a reduced cost)

($175/visit, includes any prescriptions, health coaching, or referrals needed. Routine labs will be offered at a reduced cost; must be seen every 3-6 months once established- will vary person to person.)
